Yuvo Health partners with EmblemHealth, bringing value-based care to Federally Qualified Health Centers in New York

NEW YORK — (December 12, 2023)Yuvo Health, a leading technology, operational, and administrative solution uniquely designed to give Federally Qualified Health Centers (FQHCs) an advantage in value-based care, and EmblemHealth, one of the nation’s largest nonprofit health insurers, today have announced their partnership. The collaboration optimizes cost- management opportunities and eases the administrative burden by contracting FQHCs at the aggregate level, avoiding the need for individual practice contracting.

"EmblemHealth shares our deep commitment to providing value-based care. Being able to support more FQHCs in a cost- and time-efficient manner through this partnership is especially meaningful," said Cesar Herrera, CEO at Yuvo Health. “Through our partnership, FQHCs will get credit for the value-based care they’ve been providing in their communities for decades and receive support to grow in a sustainable and financially viable way."

“We proudly work to improve health equity — both directly with our members and through their doctors — and partnering with Yuvo Health fosters a value-based healthcare model that ensures every participating health center is delivering compassionate care to their entire community,” said Melissa Vellon, Vice President Provider Performance and Reimbursement Strategy for EmblemHealth. “Our partnership aims to improve care for EmblemHealth members by focusing on their preventive care services and overall wellness, while reducing costs by incentivizing high-quality care and better health outcomes.”

Recently, Yuvo Health announced a $20.2 million Series A round of financing, enabling them to further scale from New York to Ohio, while transitioning to accept full-risk arrangements on behalf of their FQHC and Community Health Care (CHC) partners. In conjunction with their funding news, Yuvo Health added several new FQHCs, doubling their original number announced last year, which provides care to more than 150,000 patients.
