Yuvo Health partners with Public Health Solutions (PHS) to better support community health centers, address social drivers of health

NEW YORK (February 14, 2023)Yuvo Health, a leading technology-enabled administrative and managed-care solution for community health centers, today announced a partnership with Public Health Solutions (PHS), the largest public health nonprofit serving New York City. Together, the companies will combine their two ecosystems to better support community health centers and address their patients’ social drivers of health, including food, housing, and transportation.

“As a proud partner of FQHCs, we know they’re frequently asked to send referrals to community-based organizations in order to address their patients’ social needs — such as connecting them to food or transportation to get to their appointments — but they’re not supported by the system to achieve this in a meaningful and sustainable way,” said Cesar Herrera, CEO of Yuvo Health, which he co-founded in New York City with a fully BIPOC team. “Partnering with PHS, a New York City healthcare pioneer for nearly 70 years, empowers us to reach more of their FQHC partners faster, and our value-based care ecosystem provides them with the infrastructure to make this activity sustainable. It’s a win-win for everyone.”

For Yuvo Health, the partnership provides them with access to additional federally qualified health center (FQHC) partners, as well as community-based organization partners (i.e. social service providers). For PHS, which has built a coordinated network of community-based organizations called WholeYouNYC, partnering with Yuvo Health links their two ecosystems and allows community-based organizations to unlock capacity and sustainability for the FQHCs that they want to attract.

Recently, Yuvo Health partnered with the Primary Care Development Corporation (PCDC) to provide FQHCs with access to support services and new revenue streams through upside-risk contracts. Over the summer, they partnered with Fidelis Care to announce the submission of a value-based payment contract to the NYS Department of Health that supports affordable care to historically marginalized populations. Earlier this year, Yuvo Health announced four FQHCs that have joined their independent provider association (IPA): Long Island Select Healthcare, Metro Community Health Centers, Joseph P. Addabbo Family Health Center, and Advantage Care Health Centers.

As 2023 approaches, Yuvo Health will continue developing customized programs to support the unique needs of their FQHC and community health center partnerships throughout the country, starting with New York.
