Yuvo Health expands value-based care to Federally Qualified Health Centers in Ohio, scaling beyond New York with My Community Health Center (MCHC)

CANTON, OH — (November 14, 2023)Yuvo Health, a leading technology, operational, and administrative solution uniquely designed to give Federally Qualified Health Centers (FQHC) an advantage in value-based care, has entered the Ohio market, starting with its partnership with My Community Health Center, a not-for-profit, primary care health center which serves all patients regardless of their ability to pay. Significantly, as Ohio Medicaid implements its new “Next Gen” managed care program, this agreement becomes one of the state’s first examples of a value-based contract for an FQHC that intentionally moves towards downside risk.

“Partnering with My Community Health Center is especially exciting because it marks our first partnership in Ohio, expanding our innovative support model beyond downstate New York," said Cesar Herrera, CEO at Yuvo Health, which he launched in 2021 with a fully BIPOC founding team. “Additionally, MCHC will be partnering with us not just for Medicaid, but also ACO REACH, a model that promotes health equity and focuses on bringing the benefits of accountable care to Medicare beneficiaries in underserved communities, which is something we feel incredibly passionate about.”

Recently, Yuvo Health announced a $20.2 million Series A round of financing, enabling them to further scale from New York to Ohio, while transitioning to accept full-risk arrangements on behalf of their FQHC and Community Health Care (CHC) partners. In conjunction with their funding news, Yuvo Health added several new FQHCs, doubling their original number announced last year, which reaches more than 150,000 patients.

“Since launching in 2017, we have been offering comprehensive primary care services, ever-evolving from three hospital-based residency programs and always encouraging patients to be an active participant in their own healthcare,” said Juan-Paulo “JP” Afable, CEO of My Community Health Center. “Undeniably, the greater Canton community needs access to high-quality primary care and partnering with Yuvo Health enables us to not only serve the community, but to do so sustainably and in a way that facilitates better outcomes for the larger region.”
